RX Alerts

Have information to be faxed out? Please do one of the following:

 

1. Use a fax alert form supplied by the Board **call your compliance officer if you need a form**

2. Fill out an online fax alert form at: http://azpharmacy.gov/faxnet.asp

3. Call the Board office at 602-771-2744 to request a fax alert form be faxed to you.  Leave your fax number on the voice mail.

4. Print a Form from: http://azpharmacy.gov/pdfs/faxnet.pdf

5. Send an email to dwright@azpharmacy.gov and request a fax alert form be faxed to you, don’t forget to include your fax number.
